HealthCare Quality Units
Milestone operates two Health Care Quality Units as part of a long-term initiative of the Pennsylvania Department of Public Welfare, Office of Developmental Disabilities to support and improve the physical and behavioral healthcare services for citizens with intellectual and developmental disabilities. Our HCQU divisions track and monitor the health of individuals served within the state group home model and supply necessary remediation and training to provider staff to best meet the healthcare needs of individuals served.
COMMUNITY HEALTH CONNECTIONS
Community Health Connections, based in Butler, Pa. operates in Armstrong, Beaver, Butler, Clarion, Crawford, Indiana, Lawrence, Mercer and Venango Counties.
NORTHWEST HEALTH CONNECTIONS
Northwest Health Connections, based in Warren, Pa. serves the counties of Erie, Cameron, McKean, Potter, Warren, Forest, Clearfield and Jefferson.
